UC Immunization and TB Requirements

To avoid a registration hold, we strongly recommend you meet UC Immunizations and TB requirements before you arrive on campus.

 

UC Immunization Requirement

Proof of Immunization Required

Measles, Mumps and Rubella (MMR)

Two doses; first dose on or after age 1 - OR - Titer (blood test) showing immunity.

Varicella (Chickenpox)

    
Two doses; first dose on or after age 1 - OR - Titer (blood test) showing immunity. 

Note: History of varicella illness will not meet this requirement.  You will be required to perform titers showing immunity.

Tetanus, Diphtheria and Pertussis (Tdap)

One dose on or after age 7.

Meningococcal Conjugate Serogroups A, C, Y, & W-135

One dose on or after age 16 for all students under 22 years of age.

Note: Men B (Bexsero or Trumenba) will not meet this requirement.

NEW COVID-19 (Moderna, Pfizer, or Novavax)

One dose received after September 13, 2023

Influenza (Seasonal Flu)

One dose received after August 1, 2023

 

Immunizations and TB:

All incoming students including Freshman, Transfer, and Graduate Students must comply with these health requirements by the First Date of instructions of their respective Academic Term.

Students who remain non-compliant will be subject to class enrollment holds for Spring Quarter 2024. 

To ensure a safe and healthy campus community, the University of California expects that students complete ALL immunization requirements PRIOR TO ARRIVING ON CAMPUS.

General Information for Incoming Students Regarding UC Immunization and TB Requirements

  • To obtain any required vaccines, including flu vaccine, and/or TB testing at Student Health Center, appointments are required. Students are encouraged to make an appointment online at the Student Health Patient Portal or call the SHC appointment line at 949-824-5304. COVID vaccines and boosters are available at SHC. However, appointments for COVID vaccinations and boosters can only be made by telephone at this time. Call 949-824-5304 to make an appointment.

  • Students must complete the TB Risk Screening Form online via the Student Health Patient Portal prior to their appointments. 

  • Students have other options for obtaining their COVID-19, flu and other immunization vaccines including CVS, Rite-Aid, Walgreen's, Costco, Savon, Walmart and other in-network outside pharmacies and clinics. Pharmacies do not perform TB testing (e.g., titers). COVID-19 vaccines are also available at community sites. 

  • If you require a TB blood test (e.g., T-Spot or Quantiferon Gold), you may submit a request online through the Student Health Patient Portal. Click on "Messages" on the left navigation bar and follow the prompts to compose and send a secure message to SHC. Select the option: Order a Tuberculosis (TB) blood test (T-Spot) at the Lab. This type of TB test requires only one visit for the blood draw.  A lab order will be created and then you will be notified via secure message to call SHC at 949-824-5304 to schedule a lab appointment for the TB blood test.

  • Students who obtain their immunizations from an outside pharmacy or clinic are still required to enter their immunization dates and upload their records online via the Student Health Patient Portal . TB test results, if applicable, must also be uploaded. DO NOT mail, fax or deliver hard copies of your immunization or TB records to SHC as they will not be accepted. Students remain non-compliant if they have not uploaded their records; entered their vaccine dates; and/or completed the TB Risk Screening form on the portal. 

  • For students on UC SHIP:

    • No referral is needed if you obtain immunizations at one of these pharmacies. 

    • A referral is needed if you wish to obtain your immunizations or TB testing at an outside clinic other than Student Health Center (SHC). Please note that, as one common example, many CVS pharmacies have a "Minute Clinic" affiliated with the pharmacy.  Vaccines (or other services) administered by this and similar clinics, rather than by the affiliated pharmacy itself, require a referral from SHC.

    • Please note that all communications with SHC's Insurance Services department regarding referrals must be submitted via the Student Health Patient Portal in order to ensure confidentiality, privacy and security of your protected health information (PHI) and/or personally identifiable information (PII). To submit a request via the Student Health Patient Portal:

      • On the portal homepage, Go to "Messages"
      • Select "New Message" 
      • Select "SHC - Insurance: Referral Request and Other General Inquiries"
    • To send documents securely to Insurance Services, please upload the document(s) on the Student Health Patient Portal:

      • Go to "Clearances/Uploads"
      • Select "SHC Insurance Department"

Cost for Incoming Student TB Testing and Immunizations:  For students on UC SHIP, vaccines and TB testing are covered 100%. There is no office visit copay or coinsurance required. For students who have health insurance coverage through another plan (i.e. non-SHIP), the fees for TB testing and immunizations performed at SHC can be found here. Fees will be transferred to and paid directly through your campus billing account. Non-SHIP students may submit claims to their insurance carrier directly in an effort to receive reimbursement of their fees.

These requirements are satisfied by: 1) Completing the Tuberculosis Screening Questionnaire online; 2) Entering your vaccination dates online; and 3) Scanning/uploading your vaccination records online via the Student Health Patient Portal. Refer to the step-by-step instructions provided on this page and other important information pertaining to these health requirements.

Remember!

  • You are not compliant with the health requirements until you logon to the Student Health Patient Portal and complete items #1, 2, 3 above. Each item must be completed in order to be deemed compliant. 
  • You are NOT COMPLIANT with the IMMUNIZATION requirements if you simply UPLOAD your immunization records without completing the Immunization Form and TB Risk Screening Form on the portal.
  • If you are deemed to be High Risk based upon your responses on the TB Risk Screening Form, then you must either obtain TB testing at SHC or provide results from prior TB testing that occurred within twelve (12) months of entry to UCI. 
  • Please complete the Immunization Form on the portal before you upload your records.
  • New, incoming Students: You will be subject to an ACADEMIC HOLD and unable to enroll in classes for the term following your entering term if non-compliant.
  • For problems logging onto this portal, activating your UCInetID or other technical issues, please contact the Office of Information Technology (OIT) help desk at 949-824-2222 or online at https://www.oit.uci.edu/.

Students who do not show risk factors for TB as indicated by their responses on the High Risk TB Screening Questionnaire (found online on the Student Health Patient Portal) are not required to complete/submit a TB Health Assessment Form to the Student Health Center.

A Vaccine Information Statement (VIS) is a document, produced by the Centers for Disease Control (CDC), that informs vaccine recipients - or their parents or legal representatives - about the benefits and risks of a vaccine they are receiving. As a patient, you should receive a VIS from your provider for each vaccine that has been recommended for you prior to receiving the vaccine. Please visit the CDC website for the Vaccine Information Statement pertaining to the specific vaccine that you will be receiving (CDC Vaccine Information Statements) as well as additional information (CDC VIS Information).

Your vaccination or immunization record provides a history of all vaccines you received as a child and adult. This may be a "yellow card”, a paper file from your primary care provider, or a record from your previous high school or college.

If you do not have a copy of your immunization records please contact your primary care physician, childhood pediatrician, or previous high school or college to request these records. You will need these records to update your health data in the Student Health Portal, which will help you to meet the clearance requirement.

Immunization Registries: Another potential source for obtaining copies of your immunization records is your State Immunization Registry, which are secure computer systems that store immunization records.

If you experience problems related to uploading your records, please visit the Student Health Portal and follow these instructions to send a message:

  • Click on "Messages"
  • Click on "New Message"
  • Select "I would like to send a CLINICAL secure message to the Student Health Center"
  • Select "New Student Immunization Requirement" and Click "Continue"
  • Compose your message and click on "Send". You should receive a response within 3-5 business days.
